AFFECTED BY THE FAMILY OF SCHOOL REALIGNMENT 2009?

Those persons who were impacted by the Family of Schools' realignment last year and wish to return to your original family of schools must fill out and submit a Request for Transfer and Reassignment.  You may note on this application that you were affected by the realignment.  These requests are due by April 30, 2010.

REMINDER

Please inform the Assocaition and the Board if you have any changes in your personal information i.e. name, address, phone.  If you have access to the Board's VPN you can look in the HR portal under Forms> General for the appropriate form as these changes must be provided in writing.
